Fifty healthy young volunteers (22±2y) were subjected to this study. They outfitted a cuff on the upper arm from which a tube is connected to MS, OS (HEM-747IC, Omron) and a pressure transducer for simultaneous ipsilateral measurement. When measurement was performed, the cuff was inflated by a pump of OS while auscultatory measurement was performed by two skilled nurses during deflation. The cuff pressure along with oscillation waveform was continuously measured with a pressure transducer and the data was collected via a date acquisition system (Powerlab, ADInstrument). The average diastolic pressure difference between OS and MS was 4.5±5.2 mmHg. A 40% of total measurements turned out to be overestimation more than ±5mmHg. Examination of the oscillation waveforms revealed that an abrupt shrink of oscillation occurred mostly one beat before the Korotkoff fifth sound (i.e. the fourth Korotkoff sound), suggesting the greater diastolic pressure registered by OS originated at the point. Probably, the detection of the fifth Korotkoff sound for OS is achieved by an internal algorithm. Thus, it is likely that the algorithm may be influenced by various measurement conditions including stroke volume, total peripheral resistance.
