| 内容記述 | 患者から暴言・暴力を受けた精神科看護師の気分と自尊感情、精神看護への誇りについて検討した。調査は北海道内の総合病院の精神科病棟と精神科単科病院に勤務する看護師（准看護師を含む）を対象にアンケート調査を行い、有効回答を得られた337名について分析を行った。暴言・暴力をうけた経験のある看護師は289名（85.7％）であった。看護師が暴言・暴力を受けた経験の有無で有意差が認められた項目は、「年齢」、「精神科臨床経験年数」(p<0.05)、「職位」(p<0.01)であった。精神科看護師は、患者から日常的に暴言・暴力を受けており、その中でもリーダー層の看護師が多く受けていた。暴言・暴力を受けた時の気分は、「抑鬱感」と「緊張と興奮」及び「抑鬱感」と「不安感」に有意差が認められた(p<0.01)。自尊感情と「精神科看護への思い」との関連は、「自分が行っている精神科看護について誇りをもつ」と、看護師の自尊感情が高くなる傾向が認められた。 | |||||||||||||

| This study examined the feelings, self-esteem, and professional pride of psychiatric nurses subjected to verbal abuse and/or violence by patients. For statistical analysis, the frequency of and statistical values for each variable were calculated and normality was tested. A Chi-squared test was used to examine a trend for nurses who were more likely to be verbally and/or physically abused (subjected to verbal abuse and/ or violence), and one-way analysis of variance was used to examine the nurses' feelings when experiencing verbal abuse and/or violence, self-esteem, and nurses' feelings toward psychiatric nursing. Subjects were 337 nurses including licensed practical nurses working on psychiatric wards of general hospitals and in psychiatric hospitals who consented to participate in the survey. In total, 289 (85.7%) nurses had experienced verbal abuse and/or violence from patients. The presence or absence of such experience differed significantly among nurses by age, years of clinical experience in psychiatry (p<0.05), and nursing position (p<0.01). Many psychiatric nurses, especially head nurses, were subjected to verbal abuse and/or violence from patients on a daily basis. In regard to feelings when subjected to verbal abuse and/or violence, significant differences were found between "depression" and "tension and agitation" and for "depression" and "anxiety" (p<0.01). Feelings of self-esteem tended to be better for psychiatric nurses who take pride in practicing psychiatric nursing. | ||||||||||||||
